The RN Hospice Case Manager provides the primary nursing care for hospice patients, regardless of the setting. The Case Manager works closely with the attending physician and other team members to develop appropriate pain and symptom management. They complete nursing assessments and provide assistance with patient visits and admissions in various locations, as well as on-call coverage if needed. The Case Manager is to assure that patient coverage is compliant with all Federal, State and Local regulations.Job Requirements:Facilitates the transition of the patient and family into the hospice program.Assumes all on-call responsibilities as assigned.Responsible for the management of the patients’ symptoms and plan of care.Qualifications:Must be a graduate from an Accredited School of Nursing and have a current license to practice as a Registered Nurse in practicing state.Have previous hospice experience as a Case Manager.Experience in ICU/Critical care/Oncology is preferred.Must be able to maintain confidentiality regarding patient and company proprietary information.Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels.For immediate consideration please click on the "Apply Now" at the top right of this screen.At PruittHealth and PruittHealth Hospice, Committed to Caring isn’t simply a tagline, it’s the way we do business and it’s ingrained in our culture.
