JOB SUMMARY:


Assesses human responses to disease process, illness, and the death and dying process.


Identifies health care needs for the patient and their families to provide education and


an environment to assist the patient to a comfortable and dignified death. As needed,


will provide restorative, supportive and preventative nursing measures to individuals in


their home by executing the medical regimen prescribed by a licensed physician.


Fully responsible for all actions as a licensed nurse and is accountable to clients for


the quality of care delivered. Reports to his/her designated immediate supervisor


regarding work planning and assignments.


Reports to the Hospice Coordinator.


EDUCATION AND/OR EXPERIENCE:


Graduate of an accredited school of nursing required.


Current license to practice as a registered nurse in Pennsylvania required.


Minimum of 2 years clinical experience required and one year of hospice or home health


experience preferred. Chemotherapy experience and/or certification preferred.


Must hold a current Pennsylvania driver's license and have a vehicle available for work.


Requires HBV and TB vaccinations.


WORKING CONDITIONS/PHYSICAL DEMANDS:


Must be available to work on call on a rotating basis. This position involves occupational exposure to disease and illness including, but not limited to, Hepatitis B and HIV viruses.
