The clinical coordinator coordinates the day to day clinical functions for the residents on our skilled unit. The nurse works with a predominately adult geriatric population.   • Clinical: Serves as role model for staff through provision of safe, effective and efficient nursing care. Supervises clinical aspects of nursing.

  • Demonstrates knowledge of Nursing Department standards.

  • Makes rounds on all assigned units a minimum of 2 times per shift to review resident needs.

  • Acts as a concierge for skilled Unit.

  • Meet and greets all new admissions the following business day and reviews/provides consents, baseline care plans, etc.

  • Monitors implementation of nursing process on unit through observation of nursing care given. Identifies deficiencies and communicates to appropriate persons.

  • Resolves complex and difficult clinical issues as necessary.

  • Responds to all emergency situations; utilizing sound judgment, makes decisions about management of such situations.

  • Identifies and documents, according to policy, current resident problems, symptoms, and behavioral change.

  • Communicates changes in resident status to supervisor, physician, and family in a timely manner.

  • Assists with resident care as needed and monitors call light response.

  • Ensures phone calls are answered and addressed or directed to the correct person/department.

  • Ensures labs are addressed and reconciled per facility policy.

    • Leadership: Participates as an effective member of the resident care team contributing to the efficient operation of the unit.
  • Acts as charge nurse for the unit.

  • Supervises care given by ancillary nursing personnel.

  • Abides by the standards identified in FutureCare’s Statement of Corporate Ethics and the Corporate Compliance Plan.

  • Recognizes learning needs of nursing personnel on unit and provides instruction as necessary.

  • Applies effective interviewing skills to elicit necessary information from resident and family.

  • Communicates accurate information about the nursing care plan and resident status to other nursing personnel and health team members.

  • Monitors the relevance, accuracy and completes of information recorded in the medical record.

  • Reviews data obtained and documented by other nursing members.

  • Acts as preceptor for new employees as assigned.

  • Demonstrates procedures to new nursing personnel, as assigned.

  • Participated in Performance Improvement process as assigned.

  • Presents an accurate and comprehensive shift report to oncoming nurse, including resident’s condition, unit activities, actual and potential problems and possible solutions.

  • Initiates occurrence/incident reports as needed and follows through with standards of operating procedures, i.e., “neuro” checks following fall.

  • Investigates and identifies all relevant aspects of incidents as assigned.

  • Performs other duties as assigned.

    • Communication: Promotes effective relationships and communication with all staff, residents, and families.
  • Identifies and diffuses stress/conflict situations related to verbal and non-verbal communication problems with follow-up resolution.

  • Communicates pertinent information and requests through established channels.

  • Reads and encourages others to read communication books, memos, and minutes.

  • Interacts with others in a considerate, helpful, and courteous manner.

  • Maintains effective verbal communication is a professional manner.

    • Professional Development: Seeks opportunities for own growth and expansion of responsibilities.
  • Attends meetings as directed at least 75% per year.

  • Participates in education workshops/programs to increase professional competence to meet professional goals a minimum of 16 hours a year.

  • Presents a neat appearance in appropriate dress and identification as required by position.

  • Participates in self-evaluation by discussing with supervisor strengths, weaknesses, objectives, and goals.

  • Attends mandatory in-services.

    • Current LPN license
  • Progressive experience in a charge nurse capacity

  • Long Term Care experience preferred
