Nurse Practitioner (NP)

Full-time NURSE PRACTITIONER (NPs) needed.

As needed, you will be in charge of providing patients and their carers with counseling and education regarding disease processes and care management. In order to deliver seamless care across all venues, including outpatient, inpatient, and clinical research venues, the provider will be actively involved in every facet of the patients' clinical care and collaborate with other members of the care team.

RESPONSIBILITIES:

• Facilitate hospital admissions, transfers, and discharges as well as coordinate appropriate inpatient and outpatient accommodations and home-care.
• Coordinates patient care activities, based on established priorities, including teaching, counseling, and discharge planning, and directs/delegates appropriately to members of the healthcare team.
• Perform phone triage for acute clinical issues and conduct phone assessments as required per protocol and prescribe treatment as needed.
• Performs related duties & responsibilities as assigned/requested.
• Assists in development, implementation, and evaluation of standards of care and professional practice in collaboration with members of the health care team and in accordance with institutional policies and regulatory agency requirements.
  
  

Skills & Qualifications

• Board-certified in Family Medicine or Internal Medicine
• Active, unrestricted state license (NY)
• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• US work authorization
• Ability to work in a flexible environment and schedule
• Be willing to go above and beyond to give excellent customer service
• Be comfortable working in an on-site clinic environment
  
  

Pay and Benefits:

• Salary range of $150,000 to $160,000per year, based on caseload.
