Work Schedule:

• Days: Monday to Friday.
• Hours: 9:00 a.m. to 6:00 p.m., with occasional extensions until 8:00 p.m.
• Additional Commitments: Flexibility for weekend community outreach events or health fairs as required.
• Weekly Commitment: Approximately 40 hours.
  
  

Key Responsibilities:

• Conduct comprehensive health evaluations and screenings for diverse patient demographics.
• Manage acute and chronic internal medicine conditions with a focus on prevention and health advocacy.
• Collaborate with local community entities to develop and implement health education strategies.
• Provide culturally sensitive care, emphasizing patient rights and equitable healthcare access.
• Facilitate patient referrals to ensure coordinated and continuous care.
• Maintain accurate, compliant, and organized medical documentation.
• Participate in community health events to raise awareness and offer health screenings.
• Stay updated on community health trends, evidence-based practices, and relevant policies.
  
  

Qualifications:

• Valid Nurse Practitioner certification and active state nursing license.
• Master's or Doctorate degree in Nursing from an accredited institution.
• Minimum of 3 years of clinical experience, ideally in community or public health.
• Excellent interpersonal skills with the ability to engage and serve diverse communities effectively.

Compensation & Benefits:

• Salary Range: Competitive annual salary of $110,000 - $130,000, based on experience and qualifications.
