This position reviews and analyzes inpatient, outpatient, and emergency department medical records for completeness. It is also responsible for coordinating the release of information from patient medical records in accordance with regulatory requirements, hospital policies and procedures, and Federal and State statutes, including Health Insurance Portability and Accountability Act (HIPAA) guidelines.

Overview Of Responsibilities

  • Reviews and analyzes electronic and image medical records to ensure required reports are included and signatures are not missing.
  • Assigns chart completion to the appropriate providers through Epic.
  • Understands and accurately uses the deficiency tracking module in Epic.
  • Facilitates medical record completion by generating and distributing deficiency listings and compiling routine administrative and medical staff reports to support medical record completion for both Reid Health and Reid Health Physician Associates.
  • Assists medical staff and ancillary departments by retrieving electronic health records upon request.
  • Plays an integral role in the Discharge Not Final Coded process, ensuring charts are completed and coded promptly. Assists with getting charts coded and sent to Patient Financial Services for billing. Works with various hospital departments and the Director of Revenue Cycle, Coding Manager, and Operations Manager.
  • Responsible for all release of information activities for audits, attorneys, disabilities, workers' compensation claims, and insurance claims. Reviews requests for patient information to ensure proper authorization follows HIPAA regulations.
  • Works with organization personnel involved in releasing confidential information to ensure coordination and compliance with policies, procedures, and legal requirements.
  • Assists with walk-in requests for medical information.
  • Prepares and documents invoicing for requests requiring payment, following procedures for handling payments received.
  • Certifies all requested correspondence.
  • Knowledgeable about release of information guidelines for psychiatric and substance abuse patient records.
  • Stays up to date on changes in release of information regulations at both state and federal levels and implements policy changes as needed.
  • Maintains accuracy and productivity.
  • Ensures compliance with all laws, regulations, and guidelines, focusing on preventing fraud, waste, and abuse.
  • Includes other duties and projects to support department goals and expectations.
  • Actively participates in area and department meetings.
  • Attends other meetings as requested.

Education/Experience

  • Education Required: High School Diploma or GED.
  • Education Preferred:
    • Completion of a medical terminology course (preferred).
    • Associate’s degree in health information technology or another healthcare-related field.
  • Experience Preferred:
    • One to two years of clerical experience in a Health Information Management department or clinical setting.
    • Experience and knowledge in information privacy laws, access, release of information, and release control technologies, with the ability to accurately communicate guidelines to patients, insurance companies, attorneys, or physician offices.
Schedule Details

Day Shift. Monday - Friday. 8:30am to 5pm. No holidays or weekends. OT as needed.
