Description
A Greater Hope, the Inland Empire’s champion in community based services for youth and families, is
seeking a dynamic visionary to oversee our Enhanced Care Management (ECM) program. This innovative
service model is transforming the way communities support our most vulnerable populations, and we need
a value-aligned leader to bring this program to life.
The Program Manager will have program and people oversight, being responsible for hiring, training
supporting and retaining a dedicated multi-disciplinary team of care mangers to implement ECM services.
Generating, managing and assigning referrals will be a primary component of this role. This leader will
work in close collaboration with internal and external partners and the capacity to build and maintain
trusting relationships is essential.
Starting Salary Range: $140,000-$142,500
Requirements
Qualified candidates should be highly organized, hold a deep understanding of Medi-Cal populations and
have at least three years of relevant experience, in either care management, program management, or a
related healthcare field. A master’s degree in healthcare administration, social work, nursing, public
health, or a related field is desired, Active licensure in a relevant healthcare field (e.g., RN, LCSW, MSW) is
