Position: RN Care Manager
Schedule: Tuesday-Saturday with on-call rotation required.
Summary: The RN Care Manager coordinates patient care and services, ensuring quality through collaboration with patients, families, and care teams. Responsibilities include monitoring and evaluating services, facilitating safe transitions between care settings, and adhering to regulations. Candidates should have a nursing diploma, active RN license, and preferably one year of clinical experience in post-acute care.
