Responsible for planning and collaborating with managers and staff in the Head Start/Early Head Start and California Department of Education (CDE) programs to ensure the health and well-being of all children in the programs. Responsible for ongoing monitoring, record-keeping, staff training, and program implementation for health services.

POSITION SUMMARY:
Supports the work of the Y, a leading nonprofit committed to strengthening community through youth development, healthy living and social responsibility. Responsible for planning and collaborating with managers and staff in the Head Start/Early Head Start and California Department of Education (CDE) programs to ensure the health and well-being of all children in the programs. Responsible for ongoing monitoring, record-keeping, staff training, and program implementation for health services.
Essential Functions
- Track health service information in the ChildPlus data base, and conduct weekly report analysis.
- Assist the Health Manager and other staff members in generating accurate and timely health service related reports and maintain/monitor child health service outcomes.
- Meet with Family Advocates each month and assist in analyzing health service reports, identifing expired and past due health requirements, tracking related follow up and assist as needed by conducting health screenings, calling and faxing health providers, and talking to parents
- Conduct health and safety monitoring as needed; Conduct child file reviews, and provide onsite monitoring and support.
- Track, collect and enter data on developmental health screenings for children including vision, hearing, growth, hemoglobin, and blood pressure.
- Monitor health service referrals as provided for each child and ensure follow up progress reports are entered into ChildPlus data base. Monitor case notes and PIR fields.
- Monitor child’s preventative and primary health schedules using the Periodicity Schedule, including CDC immunization schedules and California PreK immunization requirements.
- Assist Family Advocates in connecting children with community health resources for medical, dental, insurance and other identified health needs.
- Work in collaboration with the Health Manager to organize the bi-annual Health and Mental Health Services Advisory Committee Meeting.
- Conduct outreach with various health service agencies to strengthen community partner relationships.
- Assist the Health Team with organizing, coordinating and implementing health and safety events across Alameda, Contra Costa and Yolo counties.
- Present the agency’s health and safety policies and procedures to large groups of staff members both in person and online; provide health-focused parent committee meeting presentations for parents.
- Support in training new Family Advocates on health services; generating health reports, interpret immunizations, complete health data entry, and follow up on special health care plans.

This section gives the minimum qualifications needed to perform the job satisfactorily.
- Bachelor’s Degree from an accredited college or university with a major in one of the following disciplines: Public Health, Social Work, Psychology, Sociology, or a related field.
- Minimum of 2 years’ experience working in a Head Start, child development program or other non-profit program serving low-income children and families.
- Bilingual in English and Spanish preferred
- T.B. and other required health clearances at the time of employment.
- Livescan/fingerprint clearance by the Department of Justice and Community Care Licensing agencies.
- Valid CDL and use of personal automobile including evidence of auto liability insurance coverage. This position requires frequent travel across service areas.
