Job Summary: The Community Health Worker serves as a patient advocate and liaison between the patient/family and community partners/program agency to the residents of Tarrant County. This position is responsible for the collaboration with community partners to ensure successful patient outcomes. This job is responsible for collaborating with community and civic leaders about Community Outreach Programs and JPS Health Network services and programs. The Community Health Worker conducts activities including recruiting participants and facilitating self-management, nutrition, psychosocial support groups, and physical activity sessions according to State approved curriculum.
Essential Job Functions & Accountabilities:
- Conveys the purpose and services of the program to the user population and the benefit to the community.
- Establishes trusting relationships with patients and their families while providing general support and encouragement.
- Conducts intake interviews with patients, including enrolling and/or referring patients into programs; assists patients with completing applications and registration forms.
- Follows-up with patients via phone calls, home visits, and visits to other settings where patients can be found.
- Assists with providing screenings regarding Social Determinants of Health and providing referrals based on resources needed.
- Conducts eligibility determination, enrollment, and follow-up with uninsured patients.
- Provides referrals for services to community agencies as appropriate.
- Helps patients connect with transportation resources and provides appointment reminders in special circumstances.
- Works closely with interdisciplinary teams to ensure patients have comprehensive and coordinated care.
- Works cooperatively with other clinical personnel assigned to the same patient.
- Provides consistent communication to evaluate patient and family status, ensuring that provided information and reports clearly describe progress.
- Ensures that all program applications submitted are complete with supporting documentation.
- Maintains a minimal error rate in accordance with established guidelines.
- Local travel may be required.

Qualifications:
Required Qualifications:
- High School Diploma, GED or equivalent.
- 1 plus year of experience in healthcare experience or community involvement.
- 1 plus year of experience creating and maintaining databases and reports for state grant funding.
- Community Health Worker Certification required or will be required to obtain certification within 6 months of hire.
Preferred Qualifications:
- Experience working in the post-acute setting (hospice, home health, nursing home, or inpatient rehabilitation hospital).
- Breastfeeding educator certificate.
