JOB PURPOSE:
The Navigator position is a combination of duties including Outreach and Case Management. The person occupying this position will work directly with Veterans that are chronically homeless and assist them in accessing community support systems, services, benefits, and permanent housing. The Navigator directly supports the homeless Veteran community and collaborates with multiple service providers, meeting regularly to triage, staff, and house the most vulnerable unsheltered homeless Veterans. The Navigator will be the lead on assisting the Veteran population with a history of homelessness. Building trust and rapport with the affected veterans will be crucial for the role.
The Navigator will have a caseload of approximately 20 Veterans and serve as the primary advocate for each Veteran for 1-2 years.
ESSENTIAL JOB RESPONSIBILITIES:
- Conduct outreach and networking activities in the community by visiting different areas such as stand-downs, encampments, bus stations, shelters, under bridges etc. Actively network at monthly Veteran community groups and provide presentations at various agencies and for community partners within the greater metropolitan and surrounding area as needed. Promote the success and reputation of the Navigator program as well as the Supportive Services for Veteran Families program.
- Conduct initial eligibility screening of Veterans seeking program assistance and complete initial intake and Case Management documentation for eligible Veterans.
- Evaluate individual and family needs and develop action plans to address goals. Complete service plans with Veterans including housing and other related needs.
- Coordinate and monitor services, including comprehensive tracking of Veteran activities in relation to service plans and Housing Inspections. Assist Veterans with securing ID, personal documents, DD214, and other records needed to access services, benefits, housing, and employment.
- Maintain appropriate records on each client, including HMIS; document services provided with date/time of each entry and include detailed case notes.
- Conduct home visits to follow up with Veterans and advocate for them. Transport Veterans to appointments as needed.
- Meet regularly with the Program Director to staff the caseload. Provide on-going program evaluation and recommendations to the Program Director for continuous growth and quality. Prepare reports as requested by the Program Director.

ESSENTIAL QUALIFICATIONS:
EDUCATION: Bachelors Degree in Social Work, Sociology, Psychology or a related field.
EXPERIENCE: 1+ years case management experience; 3+ years preferred. Experience working with the homeless veteran population with outreach efforts targeting the most vulnerable, unsheltered homeless Veteran households preferred. Knowledge of VA Programs, facilities, and community preferred.
ATTENDANCE: Must maintain regular and acceptable attendance at a level determined at the employer's sole discretion.
LICENSES: LMSW, LBSW, LMFT preferred. Driver's License with a clear record.
VEHICLE: Must have daily use of a vehicle without prior notice.
OTHER: Must be available and willing to travel to various locations as business needs dictate.
