Position Overview:
The Program Coordinator of Community Supports coordinates the following Office of People with Developmental Disabilities (OPWDD) Home and Community-Based Services (HCBS) Waiver programs: Community Habilitation, Respite, and Housing Rental Subsidy (HRS). This includes overseeing a caseload of people supported, directly supervising Direct Support Professionals (DSPs), promoting the well-being and independence of people supported, and ensuring compliance with regulations and agency policies.
- Performance Management: Provide meaningful supervision to DSPs, ensuring evaluation completion and support as needed. Review and approve employee timecards and payroll reports.
- Case Management: Oversee a caseload of people supported in Community Habilitation, Respite, and HRS, ensuring health and safety.
- Service Planning and Coordination: Schedule and facilitate person-centered meetings to develop Staff Action Plans for those supported in Community Habilitation. Coordinate and/or facilitate support meetings as needed. Record meeting minutes.
- Relationship Development: Develop meaningful relationships with people supported, advocates, and family members. Conduct spot checks and ensure satisfaction surveys are completed.
- Compliance: Ensure compliance with OPWDD regulations and agency policies, including document outreach/collection/upload and maintaining required documentation.
- Engagement: Provide direct service to people supported in need and assist with DSP recruitment and training.
- Monitoring and Follow-Up: Assist people with resolving issues that could jeopardize housing sustainability, providing updates to the support team.
- Emergency Response: Assist with implementing systems for responding to crises for people supported.
- Advocacy: Advocate for the needs and rights of people supported and assist with identifying additional resources.
- Reporting: Ensure proper reporting and investigation of incidents according to OPWDD regulations.
- All other duties deemed necessary by the supervisor.

Qualifications
- Age Requirement: Minimum age of 18.
- Experience:
- 1+ years in OPWDD HCBS Waiver programs required; supervisory role preferred.
- Educational Qualification: High School Diploma required; Associate degree in a related field preferred.
- Drivers Status: Valid Driver’s License & personal vehicle required.
- Physical Demands: Tasks may vary based on individuals supported, including physical assistance; accommodations considered for individuals with disabilities.
