Rehabilitation Supervisor

Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY. The Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish : Fay J. Lindner Residences), our Independent Living community (Fountaingate Gardens) and our Certified and Licensed Home Care Service Agencies.

Gurwin Jewish Nursing & Rehabilitation Center is currently seeking a full time Rehabiliation Supervisor to work from 7:00am to 3:00pm who will embody the passion and commitment required to deliver the ultimate care for our residents.

Requirements Include:

• B.S. from an approved school of Physical Therapy or Occupational Therapy, Masters Degree preferred
• Minimum of 3 years of Physical/OccupationalTherapy experience, plus one year geriatric related experience preferred.
• New York State License or eligibility and current certification.
  
  
  

Job Duties:

• Coordinates the management and operation of the Rehabilitation Department.
• Acts as the Center’s Coordinator of Clinical Education.
• Performs screens, evaluations and provides treatments as prescribed by the attending physician. Takes responsibility for completion of rehab sections on MDSs.
• Participates in QAPI, PDPM projects and in rehab review meetings. Provides supervision for Rehabilitation staff and volunteers.
• Assumes the responsibilities of the Director of Rehabilitation in his/her absence.
• Performs screens, evaluations and treatments under physician order.
• Maintains appropriate record keeping in accordance with facility and regulatory guidelines.
• Acts in the role of Center Coordinator of Clinical Education, liaison between the facility and affiliating institutions.
• Acts as Clinical Instructor, providing education and supervision to OTRs and COTA students.
• Responsible for the direct supervision of the daily operations of the Rehab Department, including but not limited to scheduling of residents and staff.
• Actively participates in the departmental Performance Improvement Program.
• Assists in the planning and development or revision of programs within the Rehabilitation Department.
• Assists in the revision of clinical and administrative policies and procedures.
