• rsConsults with DON, Medical Director and other members of the Care Plan Team concerning assessment evaluations, assists in developing appropriate treatment pla
• nsExamines patient/residents and their medical records/charts, recommending when to refer them to a physician for further evaluation or ca
• reAssesses skin conditions, identifying, photographing and documenting stages and any other problems upon admission/re-admission or any extended time away from the facili
• tyPerforms weekly patient skin and other examinations as needed recording results, reporting as needed and securing/applying the corresponding treatment orde
• rsEnsures that residents with decubitus ulcers receive appropriate prophylaxis and treatment, such as daily inspection, turning and activity, a well-planned diet, and maintenance of clean, dry b
