Job Summary: Provides integrated behavioral health and substance use services within primary care, focusing on prevention, intervention, and recovery. Therapists manage a caseload of patients with co-occurring conditions, offering therapeutic support tailored to individual needs.

Major Duties

• Conduct screenings and assessments, applying evidence-based practices.
• Create and implement integrated treatment plans, modifying as needed.
• Engage patients and families through outreach and referrals.
• Consult with medical providers for coordinated care.
