The Guardian Center is a short term rehabilitation and long term care center, seeking a social worker to join our team. The right candidate must possess a Massachusetts license as an LSW and have proven long term care experience. LTC experience is required.
Duties and responsibilities include and but are not limited to:

- psychosocial assessments
- completion of the MDS- mood, behavior and cognition sections
- quarterly assessments
- attend and provide input at care plan meetings
- followup on complaints and grievances
- provide ongoing support to residents and their families.
