- Perform diagnostic tests and measurements
- Develop and implement individualized care plans
- Perform continual assessments
- Coordinate with other members of patient's care team
- Provide education and hands-on instruction to patients, their care takers, and family members
- Accurately document observations, interventions, and evaluations in patient's electronic chart
- Current NJ Occupational Therapist (OT) License
- Reliable transportation and Driver’s License
- Strong interpersonal and customer service skills.
- Ability to learn / navigate electronic documentation systems (HCHB or others)
- Bilingual a plus
- Additional training certificates a plus
- Medicare Certified home care experience a plus but not necessary.
- New Licensees welcomed
