OB Nutrition Counselor

Position Summary:

As a member of Urban Health Plan's Nutrition Department and working in close conjunction with our OB/GYN Management, we are seeking an OB Nutrition Counselor to join our interdisciplinary team. This position is a fully onsite role stationed in our Simpson Pavilion in the Bronx on our OB floor. The Nutrition Counselor will conduct initial, third trimester and postpartum assessments on all pregnant women in the OB Dept. The counselor is a resource to patients that provides nutrition education to low risk pregnant women and refers high risk patients to the Clinical Nutritionist.

Key Duties

• Conducts Initial Nutrition Assessment of new OB patients.
• Conducts third trimester reassessment of OB patients reviewing weight gain, lab values and diet history
• Conducts postpartum reassessment of patients reviewing patient and infant medical and diet history
• Using established criteria, provides nutrition counseling/education to low risk patients. If patient is high risk refer to nutritionists
• Refers patients to the WIC Program and follows up on status, when applicable.
• Documents all nutrition visits in electronic medical record
• Attends and participates in meetings for OB, Nutrition and other divisions.
• Provides nutrition education handouts and presentations to patients
• Participates in Nutrition Department programs/activities (such as Health Bucks)
  
  

Minimum Qualifications

• Bachelor's Degree in Dietetics and Nutrition is required.
• Bilingual Spanish/English, required.
• 2 years of experience providing nutrition-related services.
• Team-player and willing to go the extra mile to support patients in our care.
• Proficiency with Microsoft Office and Electronic record-keeping.
• Commitment to excellence and to providing quality service to patients.
• Ability to thrive in a fast-paced environment.
• Must be organized and self-motivated.
  
  

Compensation: Minimum $52,000, Maximum $55,000.

Work Environment Type, Location, and Schedule:

Bronx based out of our OB department located at El Nuevo San Juan Community Health Center. This position will require a schedule of Monday - Friday 8:00am - 5:00pm.
