- Complete medical assessments for Independent Living candidates.
- Assist new residents with transitions and communicate needs.
- Maintain records on resident needs and provide wellness education.
- Coordinate healthcare plans and appointments.
- Support management and address resident concerns.
- Associate's Degree required; Bachelor's preferred.
- RN license and five years of related experience.
- Good communication skills and a compassionate approach.
