Position Summary
The RN Case Manager Lead performs a variety of duties centered around supporting the case management team in the management of its daily operation in the discharge and case management of patients. They have a pivotal role in ensuring the case management team operates effectively and safely transitions patients to the next level of care. This role requires the ability to show leadership, utilize critical thinking, multitasking, and problem solve issues. An additional requirement is strong communication skills which promotes collaborative relationships with the patients, their families, and the healthcare team.

Position Qualifications
Experience
- At least 2 years of experience in discharge, utilization and/or case management or equivalent combination of education and/or experience, required
- Bachelor's degree in nursing or allied health field, preferred
- RN: Registered Nurse licensed by Oregon State Board of Nursing, required
- CCM: Certified Case Manager, preferred
- ACM: Accredited Case Manager, preferred
