Description

Respond to incidents of community violence, their families, and those at highest risk for involvement.
Advocacy: Working with local government and law enforcement to advocate for policies and practices that support violence reduction and community safety
Create goal-planning processes, short-term and long-term, and service plans to promote family well-being and find new strategies to address new needs.
Collect resources on recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services
Assists clients in developing natural resources and contacting social support networks.
Provide case management services through one-on-one meetings with families to determine and meet their needs.
Assist families in applying for public benefits and services, interpretations may be needed.
Verbal or electronic translations of brochures, forms, and application instructions, as required, to access services
Review to ensure that all eligibility requirements are met, including uploading required documentation
Establish MOUs with community partners to create a referral/linkage system for clients enrolled in the program.
Community Outreach: Building relationships with community members, local organizations, and stakeholders to foster trust and collaboration

Requirements

High School, GED, or lived experiences
At least two years’ experience working with youth and families in designated communities
Must be committed to working with young people and their families who are affected by violence and have a flexible schedule including weekends and holidays.
Must have transportation, valid driver's license and insurance
