- Respond to incidents of community violence, their families, and those at highest risk for involvement.
- Advocacy: Working with local government and law enforcement to advocate for policies and practices that support violence reduction and community safety
- Create goal-planning processes, short-term and long-term, and service plans to promote family well-being and find new strategies to address new needs.
- Collect resources on recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services
- Assists clients in developing natural resources and contacting social support networks.
- Provide case management services through one-on-one meetings with families to determine and meet their needs.
- Assist families in applying for public benefits and services, interpretations may be needed.
- Verbal or electronic translations of brochures, forms, and application instructions, as required, to access services
- Review to ensure that all eligibility requirements are met, including uploading required documentation
- Establish MOUs with community partners to create a referral/linkage system for clients enrolled in the program.
- Community Outreach: Building relationships with community members, local organizations, and stakeholders to foster trust and collaboration
- High School, GED, or lived experiences
- At least two years’ experience working with youth and families in designated communities
- Must be committed to working with young people and their families who are affected by violence and have a flexible schedule including weekends and holidays.
- Must have transportation, valid driver's license and insurance
