Summary:
Promotes improved clinical data capture at the practice level to meet high priority organizational priorities and metrics. Provides clinical quality support to assure smooth and efficient patient care delivery in the medical practice or practices. Engages patients in wellness and self-care as defined by scope of role. Accountable for working with clinicians, care coordinators, and other care team members to complete pre-visit planning, chart preparation, and patient outreach to close gaps in care.
Responsibilities
Required: Data entry required.
High School diploma/GED with minimum 2 year MA experience/Front Desk staff, or Nursing Assistant/Medical training program, or 3 years job related experience in a medical office *or equivalent experience*.
Required: Basic medical knowledge and terminology. Skilled in obtaining accurate patient vital signs.
Minimum Experience: one year
Desired: Excel and EMR chronic disease registry experience preferred. Bi-lingual preferred.

Responsibilities
- Assures that the clinician has all clinically relevant documentation prior to the scheduled annual visit, including Advanced Care Planning, colon cancer screening results, breast cancer screening results, relevant results from outsides labs or providers, and other preventive health screenings and documentation.
- Works to close clinical gaps in care to improve practice-based performance on key metrics and to maximize payer program clinical quality bonus potential.
- Follows up with patients who have orders and/or referrals placed but who have missed going for important wellness screenings, lab draws, and other medical care.
- Uses motivational interviewing techniques and proactively acts as a patient advocate, responding with empathy and respect to resolve patient concerns and recognizes opportunities for improvement through patient concerns.
- Provides support on quality improvement activities at the practice level and as needed across the organization.
