Admissions Nurse (RN)

Wage: $48-55/hr

Schedule: Friday-Monday 10:00am - 6:00pm

Job Summary:

Hillsboro Health and Rehabilitation Center is looking for an Admissions Nurse (RN) responsible for ensuring a smooth and efficient transition for new residents admitted to our facility. This role involves the accurate entry of all new admission information, coordination of medical orders, and collaboration with staff to ensure quality care is provided from day one. The Admissions Nurse will work closely with hospital staff, pharmacy, and floor nursing teams to ensure all necessary processes are followed for a successful admission.

Essential Functions:

• Enter all new admission details into the electronic health records (EHR) system based on the information received from the hospital.
• Set up baseline care plans tailored to each new resident’s needs.
• Accurately enter medication, ancillary, and therapy orders as per hospital transfer instructions.
• Complete initial admission nursing evaluations for all new residents.
• Obtain all necessary consents and documentation for resident care.
• Ensure pharmacy receives medication orders for timely medication delivery upon admission.
• Greet new residents and help them settle into their rooms, ensuring they feel welcome and comfortable.
• Inform floor nursing staff about the new resident’s care plan and any specific instructions to follow for their care.
• Work closely with multidisciplinary teams to ensure smooth transitions and continuity of care.
  
  
  

Knowledge, Skills, and Requirements:

• Required: Registered Nurse (RN) in the state of Oregon.
• Preferred: Prior experience in a healthcare setting, preferably in admissions, long-term care, or related areas.
• Proficiency with electronic health records (EHR) systems.
• Strong organizational and multitasking abilities.
• Excellent communication and interpersonal skills.
• Attention to detail and ability to follow precise instructions.
