Job Title: Social Worker (Dialysis Department)

Facility: Alliance Care Rehabilitation and Nursing Center

Location: Irvington, New Jersey

Shift: Part-Time, 9:00 AM - 5:00 PM

Pay Rate: $30 per hour

Responsibilities:

• Advocate for patients by connecting them with appropriate local, state, and federal resources and social agencies.
• Plan and coordinate social service activities related to patient care.
• Attend quality improvement and patient care plan meetings and maintain case documentation.
• Counsel and educate patients and families regarding identified psychosocial issues.
• Complete annual depression screenings for patients.
• Conduct annual KDQOL surveys.
• Assess patient satisfaction and address grievances according to policies and procedures.
• Coordinate the development and maintenance of patient/family support groups.
  
  
  

Requirements:

• Education: MSW in Social Work required.
• Experience: Minimum of 2 years of social work experience, including at least 1 year with dialysis patients.
