Healthcare Staffing Professionals, Inc. is looking for a Licensed Vocational Nurse to help support the Enhanced Care Management Program. Enhanced Care Management (ECM) is a Medi-Cal managed care benefit that addresses clinical and non- clinical needs of high-need individuals through the coordination of services and comprehensive care management. Childrens Medical Services (CMS) administers the California Children Services, the Medical Therapy Program, the Child Welfare Public Health Nursing Program, and the CMS Edelman Childrens Court Pediatric Program.
ECM-CMS program will serve this particularly vulnerable ECM population of focus (POFs): Children/Youth Enrolled in California Childrens Services (CCS) with Additional Needs Beyond the CCS Condition. DPHs ECM-CMS team has extensive experience and expertise in working with this population for intensive care management and care coordination. It is anticipated that some CCS clients are also in the Child Welfare System. These clients, when encountered will be included in the initial launch. After the initial successful program launch, ECM-CMS will consider growth to fully serve both POFs. Intensive care management will be provided based on comprehensive care plans that aim at addressing the need for overall care management for the multiple systems with which the child/youth and family interact.
The positions will start immediately upon acceptance and employment clearance and are initially funded through 8/31/26 with potential renewal.

Job Duties
- The Licensed Vocational Nurse (LVN) is one of three members of the Medical Consultation Team working with the Public Health Nurse (PHN), Lead Care Managers, and Senior Clinical Social Workers in support of lead care management. The LVN will assist the PHN to integrate nursing knowledge and protocols into ECM practice. Nursing care coordination is necessary for this child/youth Population Of Focus (POF) due to their involvement in multiple health care systems. By definition, this POF must engage medical care from other than their PCPs due to their conditions (CCS-eligible condition) or status (foster care) The LVN will work with the Lead Care Managers to ensure that the enrolled child/youth POF is receiving critical recommended preventive care alongside services for their CCS-eligible condition.
- Coordinate care between multiple medical providers to help ensure that the families are accessing care for CCS conditions and recommended preventive services;
- Partner with the LCM to ensure the clients' medical conditions are included among ECM care plan goals and adequately addressed;

- Contact PCPs to ensure the clients are participating in recommended preventive care and receiveage-appropriate developmental and mental health screenings, and immunizations alongsidespecialized treatment for CCS conditions;
- Participate in medical case conferences with multiple providers to ensure holistic pediatric care;
- Consult with staff on complicated medical care coordination challenges;
- Communicate with staff across other agencies and programs (e.g., CCS, Child Welfare) to assess andsupport family engagement in care;Compile medical record information across EHR and health information exchange data sources;
- Monitor care utilization (e.g., inpatient, outpatient, pharmacy benefit);
- Educate families about the child's medical conditions and needs; and
- Answer (or contacting providers for answers) regarding clients'/families' questions about their health care
