Position Summary
The Pharmacy Referral Coordinator assists the pharmacy team to close the Coordination of Care loop by retrieving documentation from referral platforms or non-CCHS providers for revenue capture and scheduling patients referred to clinical pharmacy programs.

POSITION REQUIREMENTS
Experience: 5-10 years of pharmacy related work required. 5+ years clinical and/or pharmacy technician management experience preferred.
Licenses or Certifications: Valid state of Tennessee pharmacy technician license preferred. Pharmacy Technician Certification (PTCB) preferred.
Mental Requirements
Level 1 - Requires some concentration and normal attention. Generally, once the job is learned, the tasks can be performed more or less automatically.
Level 2 - Requires high periods of concentration intermittently and normal attention. Generally, even once the job is learned, tasks will require normal attention to deal with recurring variables.

Level 3 - Requires a high level of concentration and high level of attention intermittently. Generally, the approach to tasks may be consistent, but the number of steps required and/or the number of variables involved creates the possibility of errors unless the incumbent pays close attention.

The Pharmacy Referral Coordinator assists the pharmacy team to close the Coordination of Care loop by retrieving documentation from referral platforms or non-CCHS providers for revenue capture and scheduling patients referred to clinical pharmacy programs.
Key Responsibilities
External service coordination and documentation
- Ensure all notes are documented correctly from referring physicians and clinics to meet 340B standards
- Regularly cycle between 340b referral platforms to ensure all pending documentation is completed
- Regularly move notes from consulting providers to patient's after reviewing physician notes
- Review available time slots for Clinical Pharmacists and associated programs and schedule patients in a timely manner
- Identify appropriate resources to meet patients' needs in a timely and cost-effective manner
- Obtain insurance authorization for appointments or procedures when necessary, and act as a liaison between patients and insurance company or pharmacy as needed
- Provide information to patients about external providers as needed to coordinate effective provision of services to the patient
- Effectively communicate with Pharmacy Chair and/or Program Directors of issues and or problems
- Complete follow up activities for completed referrals and respond appropriately
- Maintain accurate and timely data for properly tracking, reporting and conducting follow-up on all of the above activities
- Greet and interact with all patients, staff and visitors in a pleasant and professional manner.
- Be knowledgeable of and in compliance with applicable standards, laws and regulations by regulatory and accrediting organizations such as: BPHC, JCAHO, CLIA, OSHA, and the State and Federal Governments. Maintain patient confidentiality according to HIPAA and the CCHS Confidentiality Agreement.
- Regularly check mail, e-mail and voicemail and respond promptly and professionally to all inquiries.
- Attend to shared responsibilities in each work area, including answering phones, cleaning, stocking, and organizing common work areas, and assisting with orientation and training of other employees as needed.
- · Attends educational trainings including conferences, webinars, roundtables as necessary
- Continue to build knowledge of the health care and pharmacy services industry, and use that knowledge to identify ways and make recommendations to improve the program.
- Work a reasonable schedule, which may include an assignment at any location and during any of the hours that CCHS provides services.
- Perform other tasks as needed, but which are not detailed within this job description. Performing other duties as required to assist CCHS in achieving its mission.

- Detail-oriented and able to extensively search for required information from physician notes.
