Case Manager Associate Pay Rate: $21.03/hour/ Case Manager Pay Rate: $51,200(Exempt) (Depending on Education and Experience)
The Esperanza Center provides a welcoming atmosphere, social services, health services, legal support and English as a Second Language (ESL) to immigrants in the Baltimore metropolitan area. The center’s staff offers assistance with employment, English proficiency, legal services, and access to primary and urgent health care and referral services. The health services are offered in partnerships with area hospitals and medical schools.
Catholic Charities of Baltimore Building of Hope is currently seeking a Case Manager Associate/Case Manager- Bilingual(English/Spanish) will provide direct service to clients and casework services that include linking clients to resources and providing referrals in our new program the Parent Child+ program, focusing on promoting early childhood development and literacy while supporting children and caregivers in Baltimore City.. Assesses individuals’ needs, connects them with resources based on goals, and provides support, advocacy, information, and referral services. Works collaboratively with team providing back-up and assistance to clients. The work schedule is Monday - Friday 8:30- 4:30pm 37.5 hours per week- This is an onsite position- Not Hybrid
This position is supported by grant funding, and the responsibilities associated with the role are governed by the terms and conditions of the grant. Employment may depend on the continuation or termination of the funding.

Job Duties & Responsibilities
- Works to maintain a safe and secure environment for clients, staff members, volunteers and visitors.
- Handles specialized tasks which include monitoring the compliance of policies and procedures, reporting incidents, completing appropriate documentation, and emergency intervention as required.
- Assists with the orientation of residents and/or volunteers.
- Provides guidance and assistance to clients by being observant and attentive to both positive and negative behavior and reporting activities to supervisor (as needed).
- Ensure residents are aware of their rights and the grievance process.
- Monitors building activity to help ensure safety, security and compliance with building policies and
- Addresses emergency situations appropriately and in a professional manner.
- Assists clients with obtaining required ID’s, Social Security Cards and shelter as required.
- Assesses severity of need for assistance.
- Complete Intakes as assigned.
- Maintains casework files and input information in the data management system within 24 hours of client contact (ClientTrack, Coordinated Access, and ServicePoint).
- Maintains logs and files related to various programs, records entries accurately reflecting events as they
- Knowledge in providing employment related services to include resume writing, job searches, and soft
- Refers clients to the Catholic Charities behavioral health team for therapy and psychiatric services.
- Provide hospitality to all visitors and volunteers and treat all clients with dignity and respect.
- Participate in meetings and training as requested, to include substance abuse, health, poverty and mental health sensitivity training.
- Perform other duties as assigned.
Case Manager Associate(204000)
- High school diploma or G.E.D. and 1 year of related experience or equivalent combination of education.
- Bachelor’s degree in Human Services, Social Work, or related field.
- Two years related experience or an equivalent combination of education and experience.
- Required to be bilingual, with fluency in both Spanish and English
- Must have ability to communicate effectively in oral and written form; organize and prioritize work; develop beneficial working relationships with service providers; pay attention to details; work independently and as a productive member of a team; and meets established deadlines and goals.
- Must be able to work flexible schedules (i.e. nights, weekends, and holidays).
- Shows reliability by being punctual; maintains consistent attendance.
- Ability to remain calm in stressful situations.
- Excellent communication and listening skills.
- Good relational skills with courteous demeanor toward staff, clients, and visitors.
- Requires the ability to utilize computer systems and software necessary to perform position functions.
- Basic Windows PC, web browsing (i.e. Chrome, Internet Explorer, etc.), and Microsoft Outlook skills Other Microsoft Office application knowledge, such as Word, Excel PowerPoint, Skype, and OneDrive desired.
- Light work that includes moving objects up to 20 pounds.
- Remaining in a stationary position, often standing or sitting for prolonged periods.
- Communicating with others to exchange information.
- Repeating motions that may include the wrists, hands and/or fingers.
- Assessing the accuracy, neatness and thoroughness of the work assigned.
- Hearing and vision abilities within normal range (corrected) to drive safely and to observe and communicate with patients.
- Driving (Agency and/or personal vehicle) is required for this position. Driver must be at least 21 years of age, with a minimum of 2 years of driving experience, and a valid driver’s license issued by the state of residence. There can be no state-issued restrictions on the licenses that would impede a driver’s ability to operate the vehicle as required by the Agency, and there must not be more than three (3) points on their driving record.
