- Collects health information and reviews biometric data to identify gaps in care and provides disease-specific education and interventions specific to gaps identified under the guidance and direction of provider, qualified healthcare professional, and/ or registered nurse.
- Interpret policies and procedures to personnel, patients, visitors, and government agencies as required.
- High school diploma or GED
- CPR certified
- CNA/HCA certified
- LPN Preferred
- One (1) year experience in a long-term facility preferred
