Job Summary

The Case Manager Non-Clinician works with individuals with mental health and/or substance use disorders to help them achieve and maintain overall wellness and independent functioning. Assessing client needs, planning and linkage to appropriate services, and advocacy are essential job functions. Coordinates and provides services within an integrated and recovery-oriented system of care.

Job Responsibilities

  • Assesses client needs and develops, with the client and the Clinical Team, treatment goals and focus.
  • Presents case review to Treatment Team, resulting in a multidisciplinary Treatment Plan.
  • Anticipates problems and utilizes strategies to avert crises. Provides appropriate crisis intervention, when necessary.
  • Provides supportive interventions to clients including direct group modality to assist clients with treatment plan goals, including involving family members, as appropriate, in the treatment process.
  • Orients clients to their level of care, their client rights, Rushford rules and regulations, and expected outcomes.
  • Collaborates with clients to develop realistic discharge plans, and helps plan for the successful implementation by clients. Completes discharged client records as per Rushford policy.
  • Utilizes community resources and makes appropriate referrals in support of the Treatment and Continuing Care Plans.
  • Maintains up-to-date and accurate client records and administrative data/reports consistent with Rushford policies that reflect the client’s course of treatment and progress toward the goals in the Treatment Plan.
  • Performs other duties as required.

Qualifications

  • Associate's degree
  • Bachelor's degree in mental health or human service discipline is preferred
  • Minimum of 2 years of previous work experience with individuals with mental health and/or substance use disorders
  • Requires current, valid Connecticut driver's license
  • Able to qualify for Public Service License is preferred
