HCHC is a dynamic Community Health Center that offers a health care delivery environment that ensures all members of our community have access to high quality primary care, dental, and behavioral health services. We also provide medication assistance support to ensure our patients have access to the lowest cost for their medications, transportation assistance to and from appointments if needed, community resource connections to close gaps with social determinants of care, integrated care models including targeted chronic disease management support with our Pharmacy team and much more. We assist patients in getting access to Medicaid as well as other insurance options using trained professional staff who serve in these roles.

Hours of operation: Monday-Friday 7:30AM-6:00PM, Saturdays 8:00AM-12:00PM

General Description Of Role And Duties

The HCHC Care Management Medical Case Manager is responsible for carrying out the mission of Healthy Community Health Centers by providing quality support services to persons infected and affected with HIV/AIDS. This is accomplished through collaborative and client-centered needs assessment completion, goal setting, supporting, and coordinating appropriate referrals to specialists and community-based resource programs to ensure access to appropriate health services, medications and other available community resources.

Requirements

Minimum education requirements are:

  • Bachelor of Social Work (BSW) or Masters of Social Work (MSW) with licensure preferred, or other related health or human services degree from an accredited college or university, or Current Virginia licensed Registered Nurse (RN) with additional Association of Nurses in AIDS Care (ANAC) certification preferred, or Related experience (HIV/AIDS or Ryan White Agency or Case Manager) for a period of two years, regardless of academic preparation
  • Knowledge of HIV/AIDS, Sexually Transmitted Infections (STIs), and behavioral risk factors for human sexuality issues
  • Knowledge of substance abuse, mental health issues and local options for treatment programs.
  • Excellent oral and written communication skills
  • Excellent interpersonal skills
  • Strong computer skills, including word processing, spreadsheets, database programs, internet.
  • Ability to work effectively with a diverse population and is knowledgeable about and demonstrates the ability to be culturally competent
  • Ability to plan, organize, and prioritize multiple tasks, accomplishing goals in a timely manner
  • Ability to work under stressful and difficult emotional situations

Additional Skills Desired

  • Bilingual/Multilingual
  • Event planning and coordination experience
  • Knowledge of medical terminology, procedures, insurance and government health programs (SSI/SSD, Medicare/Medicaid, and Department of Social Services)
  • Health Information Portability and Accountability Act (HIPPA) knowledge
