Salary: $50,480 / year

The Transition to Independence (TIP) Facilitator who works at the Service Enhanced Access Center (SEAC) of the Enhanced Young Adult Program (EYAP) provides coaching and facilitation to transitional age Youth and Young Adults (YYA) in meeting their life goals. The TIP Facilitator provides support for 10 Youth and Young Adults using mentoring, teaching skills, contingency contracting, counseling, and problem solving to maximize the likelihood of success for each person served. The Facilitator helps each Youth and Young Adult recognize that life situations provide both successes and challenges that provide valuable experiences and opportunities to learn. The SEAC TIP Facilitator will be specially trained to work with Young Adults experiencing first episode psychosis and the resources available to this sub-population in the community. The SEAC TIP Facilitator staffs the Service Enhanced Access Center and Youth and Young Adults activities associated. The SEAC TIP Facilitator collaborates with the TIP services team for requisite training and support around implementing the TIP practice model.

Responsibilities Include

  • Conducting strength-based needs assessments via the Strength Discovery Assessment process, related to the transition domains as specified in the TIP System Development and Operations Manual -- Employment & Career, Educational Opportunities, Living Situation, Personal Effectiveness & Wellbeing and Community Life Functioning.
  • Coaching and supporting Youth and Young Adults as they progress through their levels of TIP supporting Young Adults experiencing first episode psychosis.
  • Delivers evidence informed/based practices as supported by the SEAC
  • Staffs activities sponsored by the SEAC in support of the full membership of all Youth and Young Adult and Staff
  • Collects and records data relevant to the progress of Youth and Young Adult across their goals during each of the active, maintenance, and follow-along statuses
  • Facilitates each Youth and Young Adults' person-centered planning and development of his/her/their individualized TIP Core Competency Plan.
