Job Summary: The Director of Community Outreach for Hospice is responsible for developing and executing strategic initiatives to increase community awareness and engagement, fostering relationships with healthcare providers, community organizations, and stakeholders, with the expectation of increasing viable referrals to the company’s hospice program. This role drives educational campaigns, coordinates outreach programs, and supports the mission of delivering exceptional end-of-life care.

Responsibilities:

  • Hire and train all applicable Hospice staff to grow Hospice Census & increase community awareness of Hospice Care.
  • Develops and implements outreach strategies to promote hospice services and meets / exceeds budgeted targets with community organizations, hospitals & referral sources.
  • Develop annual account plans and review those plans throughout the year to monitor progress against goals and execution of account strategies. Updates account profiles, plans as needed.
  • Prepares and delivers presentations for new and enhanced programs and services to facility staff and physician office staff. Overseas implementation activities linked to such programs and services.
  • Partners with facility staff and/or physician office staff to address and problem solve issues.
  • Communicates with operational staff as necessary related to capacity constraints, complaint resolution, ability to accept cases, clinical initiatives, teamwork building, and customer inquiries as required to hit organizational goals.
  • Collaborates with marketing teams to create materials and messaging.
  • Analyzes outreach efforts and reports on ROI and community impact.
  • Represents the organization at community and healthcare events.
  • Provides training staff on effective communication with external partners.


Qualifications:

  • Bachelor's degree in healthcare, marketing, or related field; advanced degree preferred.
  • Minimum of two years of management, marketing or sales experience preferred in healthcare, nursing, social services, or related discipline
  • 5+ years in community relations or outreach, preferably in healthcare or hospice.
  • Strong understanding of hospice philosophy and regulations.
  • Excellent communication, public speaking, and interpersonal skills.
  • Proven leadership and project management abilities.


Functional Abilities:

  • Valid driver’s license may be required, as determined by operational, regional needs.
