RN/LPN - Flexible Hours - Home Health Nurses
We are seeking Part-time or Per Diem Registered Nurses (RNs) and LPNs to join our team! As a home care nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to our clients, affording them the opportunity to receive the medical care required in their homes to maintain their independence.
This position requires someone who can drive to their assigned territory. Assigned territories include North Shore Areas, Middlesex County, and Essex County.

Responsibilities:
Pay: $35.00 - $100.00 per hour
Expected hours: 10 – 40 per week

Responsibilities:
- Initiates and updates care plans with input from physicians and the healthcare team.
- Conducts evaluations to ensure patient eligibility and medical necessity per payer guidelines.
- Provides high-quality clinical services within scope and infection control standards, aligning with care plans.
- Administer medications and treatments as prescribed, ensuring adherence to safety standards.
- Monitor patients' progress and adjust care plans as necessary to achieve optimal outcomes.
- Provide education and support to patients and their families regarding health management, including home care techniques.
- Provides initial and ongoing assessments to identify the client's medical needs, level of functioning and ability to communicate and learn.
- Initiates the plan of care, necessary revisions, and performs OASIS assessments at appropriate time points when applicable.
- Initiates and performs appropriate nursing interventions.
- Coordinates services, informs the physician and other personnel of changes in the client's condition and needs.
- Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
- Prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
- Massachusetts Registered Nurse RN license required
- Homecare/VNA experience preferred
- 1+ year of experience as a Home Health Registered Nurse
- Current Registered Nurse (RN) license in good standing.
- Commitment to maintaining high standards of patient care and safety.
- Maintains a positive attitude, ready to assist patients and the agency, and is open to learning.
- Current BLS Certification
- Current Driver's License + vehicle insurance
- Access to reliable transportation
