Care Manager Supervisor

Position Summary

Assumes administrative and clinical responsibility for the day-to-day operation of the care management team.

Essential Duties/Responsibilities

  • Consistently exercises discretion and judgment to analyze, interpret, make deductions and/or additions based upon the data, and then decide what actions are necessary based on the varying facts and circumstances of each individual case.
  • Assist with establishing program goals and objectives.
  • Assists in developing, monitoring, and achieving budget/productivity for staff
  • Provide instruction and on-going training to ensure staff competencies with the electronic medical record.
  • To ensure adequate coverage for inpatient referrals and new consumers and to monitor timeliness and quality.
  • Assist, train, and monitor staff’s role in the development, implementation, and revision of the treatment plan.
  • Completion of employee evaluations.
  • Assists in hiring of new employees in accordance with current HCI hiring processes.
  • Responsibility for training of new staff in MRO service delivery and ensuring care management staff are adequately trained on a continuous basis in the protocols for billing.
  • Maintains consumer and program records in accordance with applicable standards and regulations, grant requirements, HIPPA, etc.
  • Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, boundaries, and professional stature.
  • Participates in continuing education activities, remaining knowledgeable in area(s) of expertise, with a specific emphasis on Medicaid Rehabilitation Option (MRO).
  • Attend meetings as appropriate and meet regularly with supervisor to exchange pertinent information and receive supervision.
  • Arrange or present in services to care management staff.
  • Completion of required CANS and/or ANSA training and ongoing certification
  • Monitor completion of DARMHA Supported Consumers (DSC) paperwork and reassessments within specified deadlines
  • Monitor and supervise staff training to allow progression into staff advancement
  • Meets regularly with Director to exchange pertinent information, receive supervision
  • and make recommendations for proactive process improvements, policy, and procedure changes
  • Continuously review, monitor and provide appropriate feedback on documentation
  • Satisfactory completion of all managed care trainings
  • Assist in the development and/or revision of care manager training manuals
  • Performs other duties as assigned

Minimum Qualifications/Requirements

  • Bachelor’s degree preferred with preferred majors including Psychology, Social Work, Sociology, Family/Consumer Sciences, Child Development, Substance Abuse, Education, Criminology, Counseling, and Therapy Fields. In lieu of a Bachelor’s degree minimum two years’ experience in a human services related field.
  • Knowledge and skills in community based behavioral health care and care management preferred
  • Good interpersonal and communication skills
  • Agreement to use personal transportation in work capacity including transporting consumers
  • Schedule may vary based on needs of program and/or by supervisory request
  • Acquisition and maintenance of Other Behavioral Health Professional credentials

Supervisory Responsibilities

This position directly supervises Care Managers within Hamilton Center Inc. This position will carry out supervisory responsibilities in accordance with HCI policies and applicable laws. Responsibilities include but not limited to; interviewing, hiring, and training employees. This position also includes responsibility for planning, assigning and directing work functions. Supervisor will be responsible for appraising work performance, rewarding and disciplining employees, addressing complaints, and resolving problems.

  • Maintain a valid driver’s license in accordance with HCI motor vehicle policy.
  • Maintain current Crisis Prevention Intervention (CPI) certification
  • Maintain current American Red Cross / CPR/1st Aid certification
