Opportunity

The Registered Dietitian Nutritionist provides medical nutrition therapy to Mosaic patients in the primary care setting. This position works closely with clinics to provide support, leadership and nutritional therapy interventions. Within scope of practice, position will work closely with providers and staff to serve as a resource for patient education, both individual and group, and standardization of nutrition information. This position will create and implement nutrition classes responsive to the needs of the system’s clinics and clinical processes. This position provides support for community projects or programs that help build a stronger base of nutritional awareness and engagement. This position will also provide support and guidance for development of staff wellness programs.

Patient Care and Education

  • Using evidence-based guidelines, conducts nutrition assessments and follow-up to develop patient-centered nutrition plans for patients referred by Mosaic providers.
  • Collaborates with other dietitians to support system needs.
  • Collaborates with multidisciplinary team to support the growth and development of nutrition programs.
  • Provides support for nutrition classes to collaborate, coordinate, organize and standardize for Mosaic system. Conducts classes and support groups within scope of practice.
  • Effectively utilizes team-based care model, leveraging expertise of all members of multi-disciplinary team.
  • Educates patient and caregivers using tools to effectively communicate nutrition information to support health goals taking into account culture, language, learning style and educational level.

Program Development

  • Develops nutrition related educational tools for Mosaic clinics, as appropriate.
  • Provides ongoing training for patients, multidisciplinary clinical team members and site-specific needs around nutrition.
  • Serves as nutritional resource and provides leadership across the organization for patients and staff in areas related to nutrition and wellness.
  • May precept Nutrition/Dietitian interns.
  • Identifies nutrition program development needs across the organization and develops strategies for helping meet those needs.

Documentation/Indirect patient care

  • Maintains accurate electronic health records of patients and completes documentation in timely manner using EPIC.
  • Reflects critical thinking in documentation.
  • Responds to patient telephone calls and My Chart messages in a timely manner.
  • Accurately codes patient charts to ensure correct billing.

Work Experience: A minimum of one year of experience as Dietitian in a clinical environment required. Experience providing nutrition education (individual and group) using motivational interviewing techniques to patients, families, caregivers from diverse socioeconomic and cultural backgrounds. Patient chart management through EMR/E.H.R experience required (EPIC preferred).

Education, certification and licensure: Bachelor of Science in Nutrition and/or Dietetics required. Successful candidate will be both a registered dietitian nutritionist and hold current license in the state of Oregon. Current driver’s license and insurance. Membership of the Academy of Nutrition and Dietetics required.

Skills & Knowledge: Experience working with low-income population and team-base care preferred. Motivational interviewing experience preferred. Bilingual in English/Spanish preferred, but not required. Ability to work independently in various clinic environments and willing to travel to different clinic sites throughout work week required.
