Job Summary
The Community Health Nurse (CHN) will be employed by the Community Health Center of Buffalo (CHCB) with a primary station at Heart, Love & Soul (HLS). This role involves working four days per week at HLS to address guests' health care needs, provide health education, and facilitate access to health-related services. Additionally, the CHN will spend one day per week at CHCB in a non-patient facing capacity.
Responsibilities
The role involves sitting, standing, stooping, bending, standard lifting (15 pounds), carrying, and some repetitive movements. These tasks should be executed without significant risk of injury to oneself or others, or through an explanation of how essential functions can be performed safely.

Responsibilities
- Develop and maintain positive, trusting relationships with guests.
- Coordinate and manage care for guests, including referrals, scheduling appointments, and advocating for them.
- Assess guests' needs and provide basic direct care.
- Offer on-site support services like blood pressure monitoring, medication administration, and minor wound care.
- Organize and oversee vaccination clinics, foot clinics, and other health events/services.
- Educate on healthy lifestyles, disease prevention, and managing chronic conditions.
- Assist with emergency care prior to paramedics' arrival.
- Maintain documentation of guest interactions in the designated data system.
- Collaborate with HLS Case Managers to provide comprehensive guest assistance.
- Communicate relevant information on guest needs to appropriate staff members.
- Stay informed about community support services.
- Uphold guest confidentiality.
- Manage inventory of necessary supplies.
- Attend and actively participate in regular staff and community meetings.
- Associate’s degree required, with a preference for two or more years of experience in a similar role, particularly with vulnerable populations.
- Bachelor’s degree preferred, with a preference for one or more years of experience in a similar role, especially with vulnerable populations.
- Current Registered Nurse (RN) license essential.
- Clean driver’s license necessary.
- Effective verbal and written communication skills with the ability to engage diverse audiences.
- Strong organizational skills to manage multiple priorities effectively.
- Compassionate and empathetic demeanor.
- Open-mindedness and sensitivity to diverse perspectives.
- Proficiency in computer literacy.
- Ability to educate on health-related topics.
- Willingness to perform other duties as assigned by the supervisor.

Compensation
- Pay: $32.96 per hour
