Summary:

The Mental Health Worker is responsible for providing case management services. Services include assistance with education, advocacy and providing linkages to social services such as screening for eligibility, benefits and counseling around specific needs.

In this role you will be:

  • Conducting assessments: Identifying the needs and challenges of individuals and families within the community.
  • Providing guidance: Assisting clients in navigating the application process for various benefits such as housing assistance, food stamps, healthcare, and utility assistance.
  • Advocating: Representing clients' interests to ensure they receive the benefits they are entitled to and advocating for additional resources or support when needed.
  • Connecting to resources: Linking clients with community resources, such as job training programs, educational opportunities, childcare services, and mental health support.
  • Monitoring progress: Following up with clients to track their progress, address any barriers or challenges they encounter, and adjust support services as necessary.
  • Educating: Providing information and education to clients about budgeting, financial literacy, and other skills to help them become more self-sufficient.
  • Collaborating: Working with other organizations and agencies, such as government agencies, non-profits, and healthcare providers, to coordinate services and maximize support for clients.
  • Documenting: Maintaining accurate records of client interactions, assessments, service plans, and outcomes for reporting and evaluation purposes.


Minimum Qualifications :

  • Bachelor’s Degree in Social Work, Psychology or Counseling.
  • Must Drive
  • Bilingual in one or more of the following: Spanish
  • Good interpersonal skills, ability to work with diverse social, economic, and cultural groups.
  • Proficiency in Microsoft Office.
  • Ability to plan and carry out assignments independently.
  • Ability to prioritize, adhere to timelines and multi-task.
