CenterWell Senior Primary Care Centers is seeking a dedicated Family, Internal, or Geriatric Medicine Physician to join our team. This role offers a unique opportunity to address patient needs directly in the exam room while considering the broader social determinants of health that affect health outcomes. Our innovative care model distinguishes us in providing senior-focused primary care through a multidisciplinary approach.
Key Responsibilities:
- Work Schedule: Monday-Friday, 8:00 AM - 5:00 PM; no weekends or evenings. Flexible scheduling options are available.
- Patient Care:
- See 12-15 patients per day, allowing for meaningful patient interactions.
- Serve as a lead medical expert within the Care Team, actively participating in daily huddles.
- Dedicate 100% of your time to clinical activities, including direct patient care and necessary administrative tasks (e.g., charting, meetings).
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
- Board-certified or board-eligible in Geriatric Medicine, Family Medicine, or Internal Medicine.
- Valid, unrestricted medical license in the state of desired employment (or willingness to obtain).
- Current, unrestricted DEA license.
- Experience managing a Medicare Advantage patient panel with a solid understanding of best practices in a coordinated care environment.
- Knowledge of Medicare guidelines and coverage.
- Familiarity with HEDIS quality indicators.
- Guaranteed base salary plus bi-annual bonus (20% of base).
