Job Summary:

The Activities Facilitator plays a crucial role in providing direct support and care to individuals enrolled in our new San Jose Behavioral Day Program. This position requires an engaging staff member who will assist participants with a variety of activities. The facilitator is responsible for implementing individualized program plans, fostering a positive and supportive environment that enhances the overall experience for individuals in the program.

The Activities Facilitator will collaborate with the program manager and other interdisciplinary team members to ensure the program effectively meets the individual needs of participants.

Key Responsibilities:

Direct Support:

  • Assist individuals with personal care
  • Support individuals in learning and practicing daily living skills, such asbasic cooking, cleaning up, social interaction, and community integration
  • Provide prompt assistance and supervision to ensure the safety and well-being of individuals in the program
  • Understand and properly utilize de-escalation and redirecting techniques with individuals in the program as needed
  • Participate in recreational and social activities

Program Implementation:

  • Assist in implementing individualized programs and behavior plans as directed by the program managerand interdisciplinary team
  • Update charts, document activities and observations, and provide updates to the program team
  • Maintain a safe and clean environment within the program

Collaboration and Communication:

  • Work collaboratively with program staff, managers, and other interdisciplinary team members
  • Report any concerns or incidents to the program manageror other appropriate personnel
  • Maintain open communication with families and guardians

Other Duties:

  • Assist with transportation to and from program activities, and picking up and dropping off participants at their homes as needed is preferred
  • Participate in staff training and development activities
  • Perform other duties as assigned by the program manager

Qualifications:

Education and Experience:

  • High school diploma or equivalent
  • Experience working with individuals with behavioral challenges or disabilities is preferred
  • Experience with Applied Behavior Analysis (ABA) is a plus

Skills and Abilities:

  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Knowledge of safety procedures and emergency protocols
  • Ability to adapt to changing situations and priorities
  • Basic knowledge of behavior modification principles
