CHR Consulting Serivces, Inc. is a leading health care consulting services company. We are seeking a Registered Nurse Assessment Coordinator (RNAC) to serve clients in long-term care facilities in Pennsylvania and Kentucky. The primary function of this position is to conduct and coordinate the development and accurate completion of the resident assessment and care plan process in compliance with all federal, state and local guidelines which govern the facility, and, as directed by the Director of Nursing Services and/or the Administrator so as to ensure each resident’s individual needs are identified and addressed accordingly through the assessment process.

Position requires: Current RN license with minimum of 5 years nursing experience with at least 3 years’ experience in MDS coordination and excellent working knowledge of Federal Nursing Home Regulations. Preferred candidates will have a RAC-CT certification or complete certification within six months of employment and working experience in Minimum Data Set and Case Mix review.
