Clinical Hospital Liaison (Hybrid)

Full-Time | Hybrid (2/3 remote)

Pay: $75,000 - $85,000 per year

About the Role:

Are you a connector who thrives on building relationships and helping patients transition to quality care? Our team is seeking a compassionate and dynamic Clinical Hospital Liaison to join our admissions team. This hybrid role includes hospital outreach, virtual collaboration, and in-person support for a seamless transition of patients into our skilled nursing facility.

What You’ll Do:

• Build strong relationships with hospital discharge planners, case managers, and physicians
• Identify and evaluate potential referrals for skilled nursing services
• Serve as the clinical point of contact for hospitals and healthcare partners
• Coordinate and streamline admissions by gathering clinical info and ensuring eligibility
• Represent the facility with professionalism and compassion
• Conduct in-person and virtual assessments of potential residents
• Collaborate with nursing, therapy, and administrative teams to ensure smooth patient transitions
  
  
  

Qualifications:

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) preferred, or relevant healthcare background
• 2+ years of experience in hospital liaison, marketing, admissions, or care coordination
• Experience in long-term care, skilled nursing, or post-acute care
• Strong interpersonal, communication, and organizational skills
• Familiar with healthcare regulations and insurance verification
• Proficient in Microsoft Office and EMR systems
  
  
  

Work Schedule & Flexibility:

• Hybrid Schedule: Mix of remote work, in-person hospital visits, and occasional on-site meetings at the nursing facility
