About the Role:
Are you a connector who thrives on building relationships and helping patients transition to quality care? Our team is seeking a compassionate and dynamic Clinical Hospital Liaison to join our admissions team. This hybrid role includes hospital outreach, virtual collaboration, and in-person support for a seamless transition of patients into our skilled nursing facility.
What You’ll Do:
- Build strong relationships with hospital discharge planners, case managers, and physicians
- Identify and evaluate potential referrals for skilled nursing services
- Serve as the clinical point of contact for hospitals and healthcare partners
- Coordinate and streamline admissions by gathering clinical info and ensuring eligibility
- Represent the facility with professionalism and compassion
- Conduct in-person and virtual assessments of potential residents
- Collaborate with nursing, therapy, and administrative teams to ensure smooth patient transitions
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) preferred, or relevant healthcare background
- 2+ years of experience in hospital liaison, marketing, admissions, or care coordination
- Experience in long-term care, skilled nursing, or post-acute care
- Strong interpersonal, communication, and organizational skills
- Familiar with healthcare regulations and insurance verification
- Proficient in Microsoft Office and EMR systems
- Hybrid Schedule: Mix of remote work, in-person hospital visits, and occasional on-site meetings at the nursing facility
