- One to One Care, Provide assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting, based on the individualized care plan.
- Assist with mobility, transfers, and exercises as prescribed by healthcare professionals.
- Administer medication reminders and ensure adherence to prescribed treatment plans.
- Perform light housekeeping tasks such as meal preparation, laundry, and maintaining a clean and safe environment.
- Monitor and report changes in the client's physical and mental condition to the supervising nurse or healthcare professional.
- Foster a positive and supportive atmosphere, promoting emotional well-being and companionship.
- Accompany clients to medical appointments, shopping, and other activities as needed.
- Document care activities accurately and maintain confidentiality of client information.
- Collaborate with the healthcare team to ensure a holistic and coordinated approach to client care.
- Certification as a Home Health Aide (HHA) or completion of a Personal Care Assistant (PCA) training program or willingness to obtain.
- Previous experience in home healthcare or a related field is preferred.
- Compassion, empathy, and a genuine interest in improving the lives of others.
- Strong communication and interpersonal skills.
- Ability to follow care plans and adapt to the unique needs of each client.
- Reliable transportation and a valid driver's license, if required for home visits.
- Ability to pass a background check and drug screening.
