This role requires someone who is organized, compassionate, and able to coordinate effectively with healthcare providers, patients, and families to ensure all necessary documentation is completed accurately and efficiently.
Responsibilities
- Process new patient referrals and gather required clinical information
- Verify insurance benefits and eligibility for home health services
- Coordinate initial assessments and facilitate admission paperwork
- Communicate with healthcare providers, patients, and families
- Maintain accurate and complete patient records
- Schedule appropriate care staff based on patient needs
- Ensure compliance with healthcare regulations and company policies
- Track referral sources and assist with relationship management
- High school diploma required; Associate's degree in healthcare or related field preferred
- Experience in healthcare administration, especially in home health settings
- Knowledge of medical terminology and insurance verification processes
- Excellent organizational and time management skills
- Strong communication and interpersonal abilities
- Proficiency with electronic health records and office software - training in working with Kinnser EMR is preferred.

Schedule
- Monday to Friday
- Occasional on-call rotation for Saturdays and Sundays
