Position Summary

Under the general supervision of the Family Care Program Director (FCPD), the Family Care Program Manager (FCPM) position provides day to day supervision and consultation with Lakeland Care’s Care Management (CM) Supervisors to ensure the delivery of high quality and cost-effective supports and services for the Lakeland Care membership. This position collaborates to ensure member risk is minimized for our member populations through execution of policies, procedures, and guidelines. The FCPM is responsible for ensuring that strategic planning initiatives, as well as business planning initiatives are implemented and achieved. Process development, implementation, evaluation, and improvement are critical aspects of this position to assure that proper support is afforded to CM Supervisors. It is expected that the Family Care Program Managers work as a team, and collaboratively with the Best Practice Manager to drive improvements as needed.

Responsibilities & Competencies

  • Oversight of CM Supervisors and their teams to ensure best practice is being followed and contract requirements are met.
  • Promote strong, person-centered care planning practice through consultation with CM Supervisors and interdisciplinary teams (IDT).
  • Identify practice or risk trends for both members and staff using data and in conjunction with the Best Practice Team, develop strategies to address and monitor the identified improvements.
  • Lead implementation and monitor performance of business planning strategies, and strategic initiative work within the division.
  • Maintain knowledge of Family Care contract and ensure adherence to the DHS contract requirements in practice.
  • Foster professional development of CM Supervisors, ensuring they have the tools and resources to lead care management interdisciplinary teams and achieve organizational goals
  • Provide clear and concise coaching to staff.
  • Facilitate and foster relationships with external partners (county departments, providers, community groups). Represent the organization to the community at large through professional interaction, clinical consultation, public speaking, media presentations, and participation in community advisory groups as requested.

Requirements

  • Bachelor’s degree in a Health or Human services related field, equivalent combination of education and experience (includes knowledge, skills, and abilities), OR licensed Registered Nurse in Wisconsin.
  • Minimum of one year leadership experience is required, two or more years preferred.
  • Minimum of five (5) year’s professional experience in a long-term care organization or community-based setting required. Work experience with one or more of the target groups served by Family Care preferred.
  • Knowledge of community based long term care and service delivery for the elderly, physically and intellectually/developmentally disabled adults. Knowledge of the disabilities associated with LCI target groups preferred.
  • Strong evaluation, assessment, and interview skills.
  • Strong oral and written communication skills.
  • Leadership qualities necessary to supervise and train peers.
  • Working knowledge of basic computer programs and typing skills.
  • Current WI driver’s license, acceptable driving record and proof of adequate insurance required.
