Passionate about patient care and wound management? Hickory Care is looking for a dedicated Wound Care Nurse Practitioner to care for residents in Skilled Nursing Facilities across California, working closely with facility teams to improve outcomes.

What You'll Do:

• Assess and manage acute and chronic wounds
• Develop and implement personalized care plans
• Monitor progress and adjust treatments as needed
• Document status and care for new admissions
  
  
  

What We’re Looking For:

• Current CA State RN License and Licensed Nurse Practitioner.
• A minimum of 2 years’ experience in wound care, with a solid understanding of treatment protocols and best practices.
• EMR proficiency; SNF experience and Wound Care Certification a plus
