• esAssists the Director of Social Services in the day-to-day operation of the departme
• ntAnswers phones, takes notes, coordinates transportation for patients/residents as need
• edSupports patients and families in finding resources to meet their needs through research, education, relationships with community agencies, et
• c.Provides Medicare/Medicaid and financial information when request
• edMaintains accurate social service progress not
• esUpdates the social services log and/or directory(ies) as needed or instruct
• edPlans and organizes time and duties to efficiently maintain caseloa
• dsProtects residents’ files and information per company policies and HIPAA requiremen
