Responsibilities:
- Quality Care Oversight: Ensure the delivery of high-quality care to our long-term care residents.
- Comprehensive Assessment: Conduct thorough initial interviews and assessments for new residents.
- Care Planning: Develop individualized care plans focused on both short-term and long-term goals.
- Interdisciplinary Collaboration: Lead meetings with multidisciplinary teams, caregivers, and families to coordinate resident care.
- Community Engagement: Work with specialists to optimize resident outcomes.
- Leadership & Education: Guide nursing staff, communicate effectively at all levels, and conduct in-service education on MDS, RAPS, and Triggers.
- Schedule Adherence: Maintain compliance with the MDS/Care Planning schedule for non-Medicare residents.
- Florida RN License: Valid and in good standing.
- Skilled Nursing Experience: Minimum of 2 years in a Skilled Nursing Facility.
- Expertise: Strong knowledge of MDS, Medicare, and the care-planning process.
