The Children's Case Manager is responsible for planning, coordinating,
and monitoring of services and supports provided to families and children,
receiving Family Support Services Program and children's Medicaid waivers.
The Case Manager shall have an assigned caseload of individuals for whom
he/she shall be responsible to advocate; schedule and conduct initial
assessments for eligibility for long-term care, Interdisciplinary Team
(IDT) meetings to develop and, as needed modify individualized plans,
including Family Support Plans and Service Plans (SP) for waiver
services, monitor the provision of services, supports and recommendations
identified in the FSP/SP, assist in obtaining identified services and
supports; maintain a master record and all necessary documentation; serve
as the primary contact for obtaining and disseminating information; and
facilitate and mediate conflict resolution. Inherent in all these functions
shall be activities supporting family's rights, support, empowerment,
and advoc
Requirements
Bachelor's degree required or five years of relevant experience in the
field of LTSS.
Two years' experience in the field of cognitive disabilities resource
coordination, working with children with cognitive disabilities and their
families, and/or advocacy strongly preferred.
Driving is a requirement. Must have a valid driver's license with an
acceptable driving record.

- 2 full time positions available at $25.35-$27.89/hr. DOE/DOQ.
- Approximate accrual of 15 days per year paid time off to start, paid
