Position Summary: The office of School Health (OSH) is a joint program of the New York City Public Schools (NYCPS) and the Department of Health and Mental Hygiene (DOHMH) responsible for the health of 1 million school children enrolled in approximately 1,800 public and non-public schools in New York City. Services to students include case management of chronic health problems, preventive health screening, urgent care, medication administration, preventive counseling, heath education, referral for care and assurance of ongoing effective treatment. OSH has been the provider of public health services for over 100 years.
OSH manages the health concerns of students that have the potential to impact academic success. Some cases necessitate closer tracking of services, as well as family and school guidance and support in the implementation of those services. Clinical case management can include but are not limited to assessment, care coordination, care planning, in-school visits, and referral, for optimal health and academic outcome. Case Management ensures that issues/concerns encountered or raised by students and their families are addressed. The Clinical Case Manager will provide organizational, administrative, and clinical support to the Medical Unit Performs related work.
Reports to: Medical Review Unit Program Manager.
Direct Reports: N/A
Key Relationships: Works closely with physicians, and accommodations team and DOHMH/NYCPS agents.

Responsibilities
- Facilitate seamless processing of the Medication Administration Form (MAF) reviews and accommodation services and ensure coordination of all health services implemented in schools to support students with complex medical needs.
- Perform case management students with complex clinical concerns or students that require multiple medical accommodations such as (transportation, paraprofessional, or a school nurse)
- Respond to inquiries from NYCPS staff concerning students with complex medical needs, including status of reviews and implementation of services specific to clinical support of the student.
- Work with Physician to manage daily physician reviewers’ schedules to ensure appropriate physician reviewer staffing levels, particularly during peak season for MAF submission.,
- Reach out to NYCPS liaisons/504 coordinators/Health Directors to troubleshoot outstanding coordination issues, including stablishing 504 meetings, and checking on status of implementation additional supports, e.g., para, 1:1 nurse.
- Acknowledge and prioritize emergency escalation cases identified by the Supervising Physician, or Medical Unit Leadership
- Collaborate with DOHMH/NYCPS supporting students impacted by challenging social determinants of health.
- Manage a database of prioritized cases.
- Provide proactive outreach to parents/guardians to facilitate the timely submission of accommodations paperwork.
- Troubleshoot parent/guardian concerns, provides information regarding accommodations processes, assists parental/guardian with navigation of school health policies and accommodation processes.
Minimum
- A baccalaureate degree from an accredited college and two (2) years of experience in community work or community centered activities in an area related to the duties described above; or
- High school graduation or equivalent and six (6) years of experience in community work or community centered activities in an area related to the duties as described above; or
- Education and/or experience which is equivalent to “1” or “2” above. However, all candidates must have at least one (1) year of experience as described in “1” above.
In addition to meeting the minimum requirements, individuals must have two additional years of the experience described in "1" above
Preferred
- LMSW/LCMSW.
- Clinical case management experience, medical setting preferred.
- Excellent MS Office skills (Outlook, Word, Excel, PowerPoint)
- Strong organizational and communication skills.
- Excellent customer service skills to assist in resolving any issues.
- Strong spoken and written communication skills for both internal and external communications.
- Strong attention to detail.
- Emotional intelligence.
