Job Summary
The Quality Improvement Nurse, guided by the Director of Quality Assurance, Risk Management and Compliance, supports quality improvement efforts by collecting, analyzing, and presenting clinical outcomes.
Responsibilities include EHR review, internal audits, compliance activities, data entry and ensuring adherence to standards set by UDS, HEDIS, JCAHO, Rhode Island Department of Health, AE, and organizational objectives. The Quality Improvement Nurse also assists with workflow audits for NQCA certification as a PCMH and engages in professional development to provide staff training on patient safety and quality.
The Quality Improvement Nurse collaborates with staff for quality improvement initiatives, working closely with various departments to identify areas for enhancement and implement strategies that will improve clinical outcomes. These initiatives may involve developing new protocols or refining existing procedures to ensure the highest standards of patient care and safety are maintained.
Work Schedule Demands
The position is full-time.
Due to the nature of the job, this individual must be flexible with their schedule to accommodate the client population.
May be required to work at any CCAP location.

While performing the duties of this job, the employee must frequently use of phone, writing, typing, doing vital signs and verbal communication. The tasks of this position are normally performed in an office setting, and occasionally on-site in a primary care office setting. Mobility is required to attend meetings and give presentations. The ability to travel to various locations in the state, typically via car is required. Regular lifting of up to 25 pounds is expected. Must be able to maintain a good attendance record. Requirements
REQUIRED QUALIFICATIONS:
- Active and unencumbered RN or LPN license in the State of Rhode Island
- Experience in healthcare monitoring field; preferred.
- At least 2 years of experience that is directly related to the duties and responsibilities specified.
- Experience with electronic medical records.
- Able to exercise initiative and display good knowledge of regulatory requirements UDS, NCQA, The Joint Commission.
- Provide/maintain all required immunizations and/or vaccinations
- Complete all required background checks

- Responsible for reviewing the electronic health record system (EHR).
- Develop and/or coordinate information necessary to perform crucial audits using a process-driven format with appropriate benchmarks and measurable outcomes.
- Maintain proficient documentation of QI activities.
- Collect and analyze QI data in a timely manner and summarize for presentation purposes.
- Conduct center-specific discussions with appropriate staff regarding any identified deficiencies or areas of desired performance or site improvement initiatives.
- Maintain all necessary reports for all clinical and operational quality reporting.
- Participate in the design and operationalization of Quality Improvement measures that will augment and support the overall systems of CCAP Family Health Services.
- Assist in reviewing gaps in care measures work to improve the quality outcomes.
- Participate in the Quality Improvement Committee meetings.
- Maintain strict confidentiality in all matters.
- Enliven and support the mission, vision, and values of CCAP.
- Performing other duties as assigned within scope of practice.
